Chronic care in George

Ongoing management and regular reviews of long-term conditions such as diabetes, high blood pressure, cholesterol, asthma and thyroid conditions, with a family GP in George Central who knows your history.

Long-term conditions

Managed steadily, reviewed regularly.

A chronic condition isn’t a single visit, it’s a relationship. Diabetes, high blood pressure, cholesterol, asthma and thyroid conditions all do best with the same pattern: a clear plan, steady day-to-day habits, and regular reviews to check the plan is still working.

At a review, Dr van der Nest checks the numbers that matter for your condition (blood pressure, blood sugar, cholesterol, weight), arranges blood tests where they’re due, and adjusts your plan if your health has changed. Just as importantly, there’s time to talk through how you’re actually getting on: energy, sleep, side effects, the practical business of living with a condition.

Because this is a whole-family practice, one GP can look after the household’s long-term health together, which often makes the habits easier to keep, too.
